Incentive Logic, Inc. has created this privacy statement in order to demonstrate our firm commitment to your privacy. The following discloses the information gathering and dissemination practices for our corporate web sites [www.incentivelogic.com] and [www.citysurveyrewards.com] and for Points Programs hosted on Incentive Logic, Inc. web sites that we provide as a service to client sponsors. This following privacy policy governs only our use of information gathered and disseminated on such web sites. Our client sponsors may also have a separate privacy policy governing their use of such information in which you should review and understand.

Data Collection Usage

Incentive Logic provides this Points Program as a service to the client sponsor of the Points Program. During the process of collecting contact information from Folio/reward recipients participating in the Points Program, information such as names, email addresses, shipping addresses and phone numbers are collected. The information we collect is transmitted to and stored in the U.S. We adhere to strict practices and policies concerning the collection, use, and retention of personally identifiable information received from individuals in the U.S., Europe, and other territories. For example, our privacy policy and practices conform to the Safe Harbor Principles as set forth by the U.S. Department of Commerce regarding the collection, use, and retention of data from the European Union.

All contact information collected during the shipping, information, and data gathering process is used to facilitate orders (i.e., to get in touch with a reward recipient when necessary for fulfilling a Folio/reward order). The contact, demographic and profiling information we collect from you may also be used generically to improve and customize the content of our site. However, this information is NOT shared with outside organizations for commercial purposes.

This information may also become the property of the client sponsor and is also subject to their privacy policy as provided on their web site. As a vendor to City Research, Incentive Logic, Inc. takes its role very seriously. We will not sell, rent or give access to your personally identifiable information to anyone other than the client sponsor of the Points Program. However, we may share your information under the following limited conditions:

We have your consent.
We need to provide your information to an Incentive Logic vendor who is working on our behalf to deliver your selected Folio/reward item to you.
We are responding to a subpoena, court order, or other legal process.

IP Addresses

We use your IP address to help diagnose problems with our server, and to administer our web site. In the Internet Protocol (IP), an IP address is a 32-bit number that identifies each data sender's or receiver's system.
